Discount: Professionals in the Academic, Government or Non-Profit industry may apply for a discount registration to the 2009 Global Conference. Please note that enrollment is discretionary and there are a limited number of discounts available. All discount offers are subject to approval and require a seven-day wait period. Registration: The registration area is located in the hotel lobby. Please bring this form and a valid ID to pick up your registration materials. A replacement fee of $250 will be required for all lost or stolen conference badges.

Cancellations:
Cancellations received before April 10, 2009, will be issued a refund, less a
$500 per person administration fee. All cancellations must be made in writing
and sent by e-mail to conferences@milkeninstitute.org.
Cancellations for registration received after April 10, 2009, will not be
accepted for refund. Confirmed delegates who do not cancel within the deadlines
specified above and who fail to attend will be responsible for the full
registration fee. There are no exceptions to this policy. Registrants are
responsible for the cancellation of their own hotel accommodations and travel
arrangements.
Substitutions: Substitutions within a company may be arranged with prior
notice. All requests for substitutions must be made in writing with a completed
registration form to facilitate check-in at the conference. Shared registration
is not permitted.
Attire:
Business attire will be required for all sessions, meals and functions.
Parking: Discounted self- and valet parking are available at The Beverly Hilton.
Hotel accommodations: Please be sure to make your own hotel reservations. The Beverly Hilton is SOLD OUT for the 2009 conference dates. Conference supporters and groups of 20 or more may contact Bryan Quinan, the event manager, for housing assistance: bquinan@milkeninstitute.org.
